ABOUT MY RESEARCH WORK

Epidemiology and biochemical homeostasis in breast cancer:

First phase of my research work to assess epidemiological risk factors includes menopausal status, age at menarche, age at menopause, marital status, parity, duration of lactation, age at first pregnancy, family history of cancer, domicile status, literacy, occupation, use of OCP and BMI associated with breast cancer. Genetic instability caused by decreased expression of BRCA2 could trigger mutations in DNA repair. sporadic breast cancer cases and mutations in turn leads to uncontrolled proliferation and invasive growth. Second phase includes alteration of biochemical homeostasis, abnormal hormone receptor function and abnormal sub-cellular signaling processes, as well as its predictive importance was studied. Estimation of ALP and ALT levels in serum was less sensitive than an imaging procedure, but it may be valuable for screening and early detection for metastases in breast cancer patients.

Prognostic and predictive impact of molecular factors:

NF-κB, COX-2 and β-Catenin overexpression implies aggressive tumour biology in breast cancer & it can predict tumours likely to have a poor prognosis. Patients with NF-κB, COX-2 and β-Catenin positive tumours need to be treated aggressively in our clinical management. Higher percentage of NF-κB /p65, COX-2, β-Catenin positive and CyclinD1 negative patients correlated with different type metastases or recurrence during follow-up. High levels ALP and ALT were observed in liver and bone metastatic breast cancer patients.

Implementation issues of immune targeting therapy:

Only a small proportion of patients in this study were able to afford the trastuzumab, and thereby avail of its benefits due to high cost. But, the majority of the patients were unable to afford the drug, a scenario which is representative of a significant portion of the rural Indian population. Therefore, in spite of substantial benefits of trastuzumab in the survival of patients with breast cancer, its applicability in our setup is limited and demonstrates the need for development of newer low-cost alternatives. Despite intensive clinical research, no β-Catenin inhibitors are available as therapeutic agents yet. Amongst these, inhibitors of COX-2 are being used presently in colorectal cancers, and have been demonstrated to be of use even in patients of Breast Cancer. These inhibitors have provided a low-cost option for decreasing disease progression and prolonging survival. Even though specific COX-2 inhibitors like Celecoxib have faced considerable criticism because of their cardiovascular side effects, non-specific COX-2 inhibitors like Aspirin have been used in many trials successfully without any major side effects. They can be used concurrently with chemotherapy without any adverse effects. If the role of COX-2 can be established with certainty and be incorporated into treatment regimes, it will be a viable alternative to the other expensive modalities of treatment used presently. Effectiveness of mithramycin A was overcomes the instrinsic chemoresistance and self-renewal potential of breast cancer stem cells and to sensitize them to chemotherapeutic drug doxorubicin.
